FamilyCare is searching for a full-time Certified Nurse Midwife for the Charleston, WV area. Responsible for the provision of nurse-midwifery care, including antepartum, intrapartum, postpartum, neonatal and gynecologic care. Must be available to respond in person to Women & Children's Hospital and/or FamilyCare OB/GYN & Birth Center in Charleston, WV within 30 minutes of receiving a call.

Qualifications: Certification by the American College of Nurse Midwives, the ACNM Certification Council, or a successor organization, American Midwifery Certification Board; possess a license as an Advanced Practice Registered Nurse (APRN) to practice nurse-midwifery in the State of West Virginia. Maintain current RN license for the state of West Virginia. Acquire and maintain privileges as an independent allied health professional at Charleston Area Medical Center. If eligible, maintain prescriptive authority through the WV Board of Nurses.

FamilyCare Health Centers is searching for a full-time OB/GYN physician for the greater Charleston area. Candidates will need a valid license to practice medicine in West Virginia, with a current DEA license certificate. Candidates also need to be Board certified or Board eligible.

Physician will provide the full range of obstetrics and gynecology specialty services, including outpatient visits and procedures, inpatient deliveries and surgeries.

FamilyCare Health Centers is searching for a full-time chronic care management coordinator (LPN) whom would help to oversee the comprehensive patient assessments upon entry and during the lifespan of their care. Candidates should be able to develop and implement individualized patient care plans that would be goal driven and focus on disease management education while empowering the patient to reach these goals through assisting and maintaining contact with patients and their families to make sure they have access to community resources. This role will also be responsible for helping to reduce barriers to care and ensure proper documentation of all encounters with patients per organizational policies.

Requirements:
This position requires an active, unencumbered WV LPN license of 3 years with excellent communication, telephonic and computer skills. Experience in chronic care management preferred.

FamilyCare Health Centers is searching for a Health Information Technology Specialist. This position is responsible for managing and supporting FamilyCare’s athenahealth EHR, Dentrix or other software, along with patient communications and care coordination functions.

Qualifications are as follows:
Bachelor's degree in informatics, information technology, business science or equivalent combination of education and experience or additional one-year experience may substitute education requirement, minimum of five years of health care, informatics, computer science or related field, experience working with health care data and performing comprehensive drill down analyses from both a clinical and or business perspective

FamilyCare is looking for an individual whom can comply and maintain the most up to date, accurate data for all of our providers. Including overseeing all proper credentialing and re-credentialing for providers in accordance with regulations. Being able to maintain copies of current state licenses, DEA certificates, malpractice coverage among other required credentialing documentation while also tracking these to make sure no providers have any expiration notices. Also be able to maintain provider contracts along with current health plan requirements to keep credentialing in compliance including ensuring that all practice addresses are current with health plans. Continue to maintain and oversee collaborative agreements with all providers.
